Clinical Nursing Supervisor for Homecare
Are you an experienced RN Manager of Homecare looking to join an exciting and growing team? Are you a manager who works collaboratively with other nursing supervisors and teams to ensure that the highest level of care is delivered to our patients? You will be a resource nurse working primarily in the office and occasionally assisting the field team as needed with SOC/Visits. You will also complete staff education with new and current staff, supervise visits (infection control, medication reconciliation, documentation review) and assistance with Quality Projects. You will use your expertise while monitoring and advising on high risk patients to prevent hospitalization and review daily notes to identify that skilled care and patient/family teaching is complete. Weekend rotation (1 in every 5).

This is a full time, hourly position. The schedule is Monday through Friday and does not include on call duties. Will be responsible for the following: Coordinating the Resident Assessment Instrument (RAI) and the individualized plan of care for all patients/residents. Ensuring all residents are assessed and reviewed in a timely manner using the RAI process, following MDS regulations requirements per the RAI manual, coordinating the notification of staff and family. Completing MDS’s, CAA’s, Care Plans, and resident interviews on short term rehabilitation residents. Assisting with the education of the nursing staff as well as documentation skills. Must be dependable, reliable, team player with a positive attitude and a good prior work history. Additionally, they should have MDS experience, a current MA license, LPN or RN (preferred), certification preferred but not required.
Aging Life Care Manager, MSW or RN
Looking for skilled clinician to join our growing Aging Life Care Management practice. Provide client-centered care management service to elders and families including: in-home assessments; develop and monitor care plans; consultation to families; advocacy for elders in institutional and home settings. Some administrative functions are part of this position. Must have excellent clinical skills, experience working with elders and families, strong writing ability, comfort with computer use. Must be self-starter and have ability to work independently. Informal yet highly professional work environment. Part time with potential to growth. Required experience: Post Degree employment: 3 years. Required license or certification: MSW or Holistic RN; will consider BSW with strong experience.
Cook Supervisor – Full Time – JHC
Must be proficient in quality, large volume, scratch cooking and supervising other food service workers. Looking for dependable, flexible, organized, efficient, skilled individual with good prior work history. 2 years of experience in a healthcare facility preferred. This is a 40 hour position, 11am-7:30pm. Schedule includes every other weekend and some holidays.
Diet Tech – Full Time
Hours are 10am-6:30pm and 6:30am-3pm 2x’s per week and every other weekend. Under the direction of the Clinical Dietitian you will be required to perform a variety of duties related to clinical nutrition (menu selection, correction menus and working on tray line). Good customer service and communication skills preferred. Must be extremely organized, flexible and dependable.
Dietary Aide – Full Time

Per Diem position available in our Senior Living Residence. Assist with creation, coordination, and implementation of activities to meet needs, capabilities and interests of residents. Conducts individual and group activities. Escorts and supervises trips and special functions.
Administrative Assistant – Part Time
Flexible schedule, 2-3 mornings a week, 5-10 hours a week. Duties include setting up client files, entering sensitive information into computer, compose and type correspondence, assisting in on-line and phone research, scanning and managing resource database, filing. Be willing to attend appropriate training programs in and out of the office. Successful candidates will have 6 -12 months of related experience or training and be proficient in computer use including word processing and data entry. Medical Scheduler for Homecare – 16 hours
We are seeking a part time scheduler to join our dynamic HomeCare team, hours are every Friday and Saturday OR every Saturday and Sunday, 8:30am-5pm. Responsible for scheduling visits to patients, ensuring that the visits are scheduled at appropriate times and intervals, with the appropriate clinician, to meet the MD ordered Plan of Care. Re-assigns visits as needed to cover staff call outs and urgent clinical needs as directed by clinical supervisor.
