RN Case Manager – PT (24 hrs) 8am-4:30pm
Acts as liaison between resident and family social worker, third party payors, rehab. referring agency (Home Care) and physicians. Evaluate, and manage interdisciplinary plan of care; ensure proper documentation for planning, implementing and discharge planning. Implement and oversee resident/family teaching. Plan, coordinate, and facilitate the Medicare meeting (document problems, progress toward goals, set anticipated discharge date.) Continuously evaluate and implement appropriate changes (CQI) to best meet the patient’s needs. Provide for continuity of care for all residents with high standard of quality care. Make room visits and lead rounds with the IDT. Be flexible in work schedule to meet department needs. Ideally you should also have strong organizational, prioritizing and analytical skills and a working knowledge of computer and software applications and are a strong team player with a positive attitude, with a positive prior work history and great references. Required: Current MA RN License, reliable and flexible, case manager experience
Speech Therapist – Homecare – Part time
We are currently looking to add a part time Speech Therapist (flexible 15-20 hours per week) to join our team to work within the Homecare team and with patients/residents of Jewish Healthcare Center. You will be responsible for evaluating residents and developing appropriate resident specific treatment plans specific to the resident’s diagnosis and type/age of resident. Current MA SLP License and must be or become CPR certified within 6 months of employment and maintain certification. Knowledge/experience in working with the elderly preferred. Reliable transportation and valid driver’s license for reasonable local travel
MDS/Care Plan Coordinator – Supervisor
We are looking for a MDS Coordinator – Supervisor to work full time, days, Monday-Friday. In this role you will supervises MDS/Care Plan Coordinators. Duties include: Select, orient, train, guide, develop and motivate staff to excel; Responsible for the coordination of care plan meetings and schedule of MDS assessments; Ensure all residents are assessed and reviewed in a timely manner using the RAI process; Follow MDS regulation requirements per RAI manual; Coordination includes notification of staff and family; Teach clinical staff how to improve assessment and documentation skills to enhance the quality of care delivered; Participate in review of nursing documentation to teach staff and to ensure that they reflect patient/resident individualized plan of care in their documentation; Review care plans to ensure that each patient/resident has a comprehensive care plan that is individualized and realistic, with measurable goals and approaches; Periodically attend care planning conference; Responsible for coordinating the Resident Assessment Instrument (RAI) and the individualized plan of care for all patients/residents and for Case Management. Requirements: Registered Nurse; have IV certification (peripheral and central), be CPR certified; at least two (2) year’s experience in a long term care setting.
Homecare Admissions RN
Our HomeCare program seeks experienced Homecare admissions nurse to perform an at least 3 admissions per shift in the greater Worcester area. This is a 24 hour (pay for 30) Baylor position, 1st shift Saturday, Sunday. Some holiday coverage required. Must have current RN MA clinical license and reliable transportation. Exp. in Homecare and doing home care admissions required. Knowledge/experience in working with the elderly preferred. Organized and dependable. Reliable and flexible with the ability to work autonomously. Comfort level with electronic documentation with basic computer knowledge.
Resident Associates – HHA or CNA
We are looking for Resident Associates to work with our dedicated Assisted Living staff. HHA or CNA license required. Per Diem. Responsibilities include direct care, transporting patients to planned activities, passing meal trays, making beds and assisting our nursing staff.
Homecare LPN/RN – Full Time
This is a full time position Monday – Friday with holiday and weekend rotations. Must have current LPN or RN, MA clinical license and reliable transportation. EXPERIENCE IN HOMECARE PREFERRED. Knowledge/experience in working with the elderly preferred; strong team player with a positive attitude; organized and dependable; reliable and flexible with the ability to work autonomously; positive prior work history with great references; comfort level with electronic documentation with basic computer knowledge. Duties include: Perform ongoing assessments of patient needs; develop and implement necessary measures/interventions to implement the plan of care; provide care and services based on the assessment of the patient’s needs and identified in the plan of care; evaluate response to interventions and the effectiveness of the plan of care. Provide patient, family and primary care givers with education and training as appropriate regarding the care and services specified in the plan of care. Assist in obtaining durable medical equipment, and drugs and biologicals related to the management of the illness and related condition. Participate in assigning duties, and providing instructions for care and services, directing, supervising, and educating other members of the Homecare staff and assess the skills and competence of individuals providing care and services.
CNAs – Full, Part Time & Per Diem

Prefer 5 years RN exp, with a least two years experience in a long term care setting, with one year being in a supervisory capacity. General knowledge of regulations pertaining to nursing care in long term care (OBRA, OSHA, State and Federal Regs.) including how they apply to residents, staff and the facility. Must be IV certified.
Recreational Therapy Assistant

Pot Washer / Utility Person – Full Time
Our Assisted Living facility has an immediate, full time (40 hours) position. Perform a variety of duties related to washing and storing of pots and pans and other food service equipment used in the preparation and service of meals in accordance with the Orthodox Jewish Dietary Laws. Duties include: wash, sanitize, and store pots, pans, and utensils. Clean kettles, grills, ovens, steamers, and exhaust hoods as needed. Maintain cleanliness of assigned areas; including hanging carousels, pot racks, pot sinks and adjoining areas. Must be able to occasionally lift and/or move up to 50 pounds. Schedule is as follows: 1st week: Sun. off; Mon.; Tues.; Wed.; Thurs.; Sat. 11:30am-8pm.; Fri. off; Sat. 7am-3:30pm. 2nd week: Sun. 7am-3:30pm; Mon. off; Tues.; Wed.; Thurs.; Fri; 11:30am-8pm; Sat. off.

Looking for an experienced housekeeper. This position requires every other weekend. Hours are 6:30am-3pm. Schedule includes every other weekend. Must be reliable, dependable, flexible, team player, with a positive attitude and a good prior work history. Ability to communicate and interact with patients/residents is required. Prior housekeeping experience in health care setting required.
Diet Aide / Food Service Worker
We have two full time positions open. Both are 6:30am-3pm. One is 32 hours, the other is 40 hours. Schedule includes every other weekend. Duties will include setting up and working the tray line. Maintaining work stations in a clean and sanitary manner throughout the shift. Washing dishes, pots, and pans. Clean, sanitize and store food trucks, transport carts and equipment after each use. Date and label all foods according to department procedures. Protect food from contamination by using proper food handling procedures during preparation, storage, service and transportation. Must be caring, dedicated, flexible and a great team member.
