Director of Nursing – up to $2000 Sign on Bonus

Competencies and Requirements: Works directly with Director of Clinical Services to maintain up to date clinical policies and procedures; Must possess a current MA RN License and a minimum of 2 years’ experience in an elder/geriatric long term care setting as a Director of Nursing or Assistant Director of Nursing; Must be an excellent leader and role model to all; Possess a positive, caring, proactive, results oriented approach; Must develop, adhere to and support policies and procedures; Must possess in depth knowledge of regulations pertaining to long term care (OBRA, OSHA, State and Federal regulations, etc.) and how they apply to residents, patients, families, staff and the facility; Will be responsible for the development of objectives and standards of nursing practices and procedures, and overall compassionate leadership for all levels of nursing staff. Charge Nurse (RN or LPN) up to $2000 sign on bonus!
This is a full time position, 2:30pm-11pm, with on-call coverage some holidays and weekends. In this role you will have an integral role in providing comprehensive nursing care to patients after acute hospitalization. Will demonstrate a high quality of care to the patients, will delegate nursing tasks to the licensed nurses/nursing assistants on the unit and excel in providing customer service to patients, families and visitors. Responsibilities: Daily rounds on unit to assess patient; Review new admission charts with appropriate follow-up as needed; Review new orders from previous 24 hours for accuracy; Enter new orders; Weekly wound assessment/measurements; Review patient documentation for discharge (md order, discharge referral complete, education/teaching completed); Ensure completion of education required for safe discharge/identify barriers to education and collaborate with interdisciplinary team; Ensure staffing is in place in dining room on unit-Ensure dietary recommendations, daily labs, psych recommendations are reviewed by MD/NP with appropriate follow through; Update care plans with change in status or with post incident interventions; Assist with medication dispensing and treatments as needed. Requirements: Must be a Registered Nurse or Licensed Practical Nurse with current Massachusetts nursing registration and be IV certified of peripheral and central lines, or become so within six months of employment and maintain certification through annual education related to IV therapy. Must be or become CPR Certified within 6 months of employment and maintain certification. Preferred: Two (2) years’ experience in long term, with one (1) year experience in a charge nurse role; have general knowledge of regulations pertaining to nursing care in long term care and how they apply to residents, staff, and the facility. Our in home private care division is looking for CNAs or HHAs to provide in-home private care and assist in our Recreational Therapy department. This position requires every other weekend. All days, all shifts available. $1500 Sign on bonus available for full time (32+ hours).These positions require some local travel to patient homes around the Worcester area and ability to work in the facility as well. Your own reliable transportation is required.
Dietary Aides – Part time, 4pm-7:30pm
We are looking to add to our caring and professional food service team. Prior food service experience preferred but not required. We are looking to add to our caring and professional server team. We have immediate part-time openings, 4pm-7:30pm (schedule includes every other weekend). Prior experience preferred but not required. High school students (and others) welcome to apply! Duties will include setting up and working the tray line. Maintaining work stations in a clean and sanitary manner throughout the shift. Washing dishes, pots, and pans. Protect food from contamination by using proper food handling procedures during preparation, storage, service and transportation.
Housekeeper (first shift/full time)
Full time position for experienced housekeeper in a healthcare setting. This position requires every other weekend. Hours are 6:30am-3pm. Must be reliable, dependable, flexible, team player, with a positive attitude and a good prior work history. Ability to communicate and interact with patients/residents is required. Prior housekeeping experience in health care setting preferred.
Maintenance Assistant – Plant Services
Full time position, Tuesday-Friday 7am-3:30pm, Saturday 12pm-8:30pm (40 hours). Assist in performing general maintenance and preventative maintenance, repairs, and painting and patching; Perform seasonal grounds work and assist with winter snow removal; Change memorial plaques according to established schedule; Prepare rooms/areas for functions; Ensure building and entrances are secure. Assist in monitoring heat, air conditioning, lighting and entrances; Request supplies as necessary to perform duties. Six months experience in a health care setting is preferred. Must be able to frequently lift and/or move between 20-50 pounds and occasionally lift and/or move up to 100 pounds. Must have valid MA Driver’s License.
Medical Supply Clerk – Part Time
This is a 20 hour per week, first shift position, hours are Monday, Wednesday and Fridays 7am-2pm. Performs a variety of functions related to purchasing, receiving, inventorying, transporting, and dispensing of supplies, and also a variety of clerical duties associated with supply management. Ability to communicate effectively both orally and in writing, good telephone manner, and ability to operate basic office equipment (fax, copier, computer) required. Must be able to work independently and as part of the team, frequently lift 25 pounds, and occasionally lift, push and pull up to 50 pounds. Successful candidate will also have positive attitude and good prior work history.
Receptionist – Every other weekend OR Per Diem
We have two positions – one is part time, every other weekend (both Saturday and Sunday). The other is Per Diem. We are looking for an experienced receptionist, responsible for answering phones, greeting guests and for performing other clerical tasks as needed. Successful candidate will be dependable, accurate, flexible, organized, detail oriented, and able to multi-task while working with a positive attitude and have a good prior work history. Good attendance is a must!

Social Worker – Full Time
Assure that medically related emotional and social needs of residents are met/maintained on an individual basis. Become a liaison to the community and to health care providers to ensure awareness and knowledge of programs and services of the Center to maximize client base. Responsibilities: Assist residents and families through intake and admission processes and adjustment to the Residence. Advise them of personal and property rights of the resident, and items which are allowed to be brought into the Center. Provide information regarding payment sources and obligations and advise of available sources of payment. Provide counseling to meet psychosocial needs of residents and families (Validation, granting permission). Refer patients and residents to a physician for Med reviews or counseling. Participate in resident care meetings to develop and implement inter-disciplinary care plans to meet the psycho-social and medical needs of residents. (Advance directives, comfort-care). Complete documentation, in timely manner, as required, including documentation for the medical record (assessment/history, quarterly notes, progress notes and discharge). Keep supervisor, Director of Nursing and Clinical Services and fellow social workers informed about case load, census activity, transfer notices, and about difficult patients, residents & families. Develop and maintain effective working relationships and communication with other departments within the facility. Participate in orientation and in-service training programs. Recommend changes necessary for the Department. Participate in Continuous Quality Improvement program. QUALIFICATION REQUIREMENTS: Massachusetts State license to practice Social Work with a MSW preferred. Minimum 1 to 2 years of experience in a skilled nursing facility preferred. Prefer experience with geriatric population.
