Short-Term Rehabilitation

-

How Can We Help?

  • Do you feel you need more help before returning home after hospitalization?
  • Does your physician recommend short-term rehabilitation to increase strength before returning home after your surgery or hospitalization?
  • Are your supports limited at home making return from surgery or hospitalization more difficult?
  • Is your spouse/caregiver also frail and/or has health issues making caregiving difficult following hospitalization?

We understand the challenges that patients, loved ones and families face after an acute illness, surgery and/or hospital stay. Our goal is to help you recover and recuperate as much and as quickly as possible to ensure that your return home is safe and successful.

The Short-Term Skilled Nursing and Rehab program offers exceptional nursing care for the acutely ill, as well as short-term rehabilitation. We are dedicated to rebuilding health and restoring maximum possible function.

Our nursing and rehab professionals create a customized and thorough plan for each loved one to ensure the best possible outcome. Our strategic rehab programs, which mimic the home setting, promote greater success.

Does one have to be Jewish to utilize the services of the Jewish Healthcare Center?

The Jewish Healthcare Center is proud to provide care to all, regardless of race, religion or cultural identity

Can I eat non-Kosher food even though the dining areas are Kosher?

Residents are welcome to eat non-Kosher food in the comfort of their own rooms. Because the dining areas are Kosher, non-Kosher dining is only permitted in one’s private room and in designated community areas.

How are religious needs addressed?

We ask what the resident would like to fulfill his or her spiritual needs. Our campus has a full-time Chaplain who leads a weekly discussion group, a variety of religious services, and provides private pastoral support as desired. Catholic services, mass and communion are also provided.

How many days per week is the therapy department open?

To help patients regain their health and reach their full potential as quickly as possible, the rehab department is open seven day per week. Every patient receives an individualized treatment plan, based on his or her condition and specific rehabilitation needs. JHC will provide the most appropriate services as recommended by the therapy team after admission.

What therapies are available at the Jewish Healthcare Center?

Our in-house staff includes highly skilled occupational, physical, speech therapists, wound care nurses, case management, social workers, dieticians, and recreational therapists. Our expertise includes neuro-rehabilitation, cardiothoracic, orthopedics and wound care.

How do I get a referral?

Speak to your hospital case manager or primary care physician to ask for a referral.
