Assure that medically related emotional and social needs of residents are met/maintained on an individual basis. Become a liaison to the community and to health care providers to ensure awareness and knowledge of programs and services of the Center to maximize client base. Responsibilities: Assist residents and families through intake and admission processes and adjustment to the Residence. Advise them of personal and property rights of the resident, and items which are allowed to be brought into the Center. Provide information regarding payment sources and obligations and advise of available sources of payment. Provide counseling to meet psychosocial needs of residents and families (Validation, granting permission). Refer patients and residents to a physician for Med reviews or counseling. Participate in resident care meetings to develop and implement inter-disciplinary care plans to meet the psycho-social and medical needs of residents. (Advance directives, comfort-care). Complete documentation, in timely manner, as required, including documentation for the medical record (assessment/history, quarterly notes, progress notes and discharge). Keep supervisor, Director of Nursing and Clinical Services and fellow social workers informed about case load, census activity, transfer notices, and about difficult patients, residents & families. Develop and maintain effective working relationships and communication with other departments within the facility. Participate in orientation and in-service training programs. Recommend changes necessary for the Department. Participate in Continuous Quality Improvement program. QUALIFICATION REQUIREMENTS: Massachusetts State license to practice Social Work with a MSW preferred. Minimum 1 to 2 years of experience in a skilled nursing facility preferred. Prefer experience with geriatric population.
